Insurance & Payment FAQ

I accept some insurance plans and many clients use their health insurance to cover their appointments!

Coverage varies between plans and between states. Please scroll down for help determining what your plan covers. I have years of experience billing insurance and I work diligently to make sure your visits are covered, most often without a copay.  If insurance declines to cover your visit(s) for any reason, you are responsible for payment.

I have listed some information below regarding insurance coverage for nutrition therapy based upon my knowledge and/or experience to date with the companies. Please understand that we cannot know the details of each plan and that the information below is not a guarantee of coverage. I do not regularly check benefits for clients so it is your responsibility to know what your plan covers and what your portion of the cost is. Yes, I am an in-network provider with several major insurance plans and also offer superbills for out-of-network reimbursement. Coverage varies by plan and diagnosis, so it’s important to verify your benefits before beginning services.

What CPT codes are used for nutrition services

We bill using the following medical nutrition therapy (MNT) CPT codes:

  • 97802: Initial assessment and intervention, individual, face-to-face (used for first appointments)

  • 97803: Reassessment and intervention, individual, face-to-face (used for follow-ups)

These codes are recognized by most commercial insurance carriers and some state Medicaid plans, depending on your diagnosis and policy.

What are your private pay rates?

For clients not using insurance or whose plans do not cover nutrition services, the following rates apply:

  • Initial Session (97802) – 60 minutes: $200

  • Follow-Up Session (97803) – 45 minutes: $150

Payment is due at the time of service. Superbills are available upon request if you would like to submit for out-of-network reimbursement.

How can I find out if my insurance covers these services?

To check your benefits, call the customer service number on the back of your insurance card and ask:

  1. Do I have coverage for medical nutrition therapy (MNT)?

  2. Are CPT codes 97802 and 97803 covered under my plan?

  3. Is Randi Beranbaum, MS, RD an in-network provider?

  4. Do I need a referral or diagnosis code to qualify for coverage?

Common diagnosis codes that may be covered include PCOS, diabetes, high cholesterol, underweight, and eating disorders. Preventive nutrition counseling is sometimes covered as well.

Do you offer payment plans and sliding scales?

Yes. If cost is a barrier, I am happy to discuss flexible payment options. My priority is making care accessible and sustainable.
